The certified report submitted MUST still be POSTMARKED by the 14th or hand delivered by the 15th.

The date that the report is completed online has no bearing on reporting deadline requirements.

Any report that does not include the certification number or submitted after these dates will be considered delinquent.

Only one remittance is now required:

Make check payable to: Phoenix Electrical Receiving & Administration Fund for the amount equal to the total contributions for NEBF, Health & Welfare, Pension/Annuity, Apprenticeship, NLMCC, LMCC, Substance Abuse Program, Phoenix Electrical Receiving & Administration Fund, NECA Dues (if the employer is a member of NECA) and Local 640 Union Dues.

Forward with five (5) copies of the completed payroll report and attachments to:

Reports postmarked after the 14th or hand delivered after the 15th will be considered delinquent thus resulting in additional penalties. Liquidated Damages are assessed for failure to file monthly report (even if there are no hours or contributions to report) or failure to pay the appropriate amounts.

The employer should retain a copy of the completed report.

IMPORTANT— Only employees who are covered by the bargaining agreement and employees covered pursuant to Article 6 of the Restated Employees  Benefit Agreement and Trust for the National Electrical Benefit Fund should be entered on the report.
